Speaking personally, as someone who owns a business in the state of Virginia and does not have a business that's large enough to be required by law to provide insurance to my employees, and isn't generally profitable enough to be able to afford to offer it. My annual income is basically only what I need to survive, as such it's low enough to be considered 'poverty level'. In Virginia, because my income is considered poverty level, and my state does not extend Medicaid, I do not qualify for any tax credits. As a result, my insurance costs are going to be increased. My provider has already sent me a notification of my policy rate changing effective 2014. There's no cheaper option available either.

However, for fun, I looked into what it would be if I were in Maryland still, and under the exact same circumstances, my insurance costs would decrease several hundred dollars instead, and I'd be able to go with Medicaid if I wanted to (decreasing costs further). So, because Virginia decides not to extend medicaid, and the providers in this state know it, they're increasing their rates on people.
