There's a small trick with hidden sheaths I've discovered.

If you wear something that will hide the sheath, wear the sheath, then remove what was hiding the sheath in the first place, the sheath will remain hidden. So long as you don't remove or pull the sheath into view, it'll remain hidden even if you put your knife in it.

I've managed to wear hidden sheaths on my ankle with sandals, even though only boots will hide them, for example.

Hidden sheaths will also hold other items besides knives. I've stuck dagger fans in them before when I was giving a fan dance because I loved the image of pulling fans out from under my sleeves.
