Literally it is past tense for wake. Wake is to come out of a deep sleep. So it describes having come out of a deep sleep.
What does it mean to you? When you hear it what comes into your head?
When I first heard it, I understood it to mean that the "woke" person was seeing clearly and not being misguided by the many misinformation efforts, whether from religious, political, or other sources. Able to see through the propaganda and able to understand what is the truth.
Edit: Since then it has been broadly used and abused and I no longer think it means that when I hear it. It has become somewhat meaningless.
