Despite being on Cracked, this article makes a lot of good points about the reasons and groups and such. It's an interview with David Kilcullen, former Chief Strategist in the State Department's Office of the Coordinator of Counterterrorism. Every now and again Cracked does some actual journalism, even if it usually does lean left:

This is why I focused particularly on the infrastructure. Certainly there are Nazis in our military - it is impossible to select a group of a million Americans without getting a couple Nazis. But to graduate from an insurgency to a bona fide civil war requires more than some rifles and fertilizer, and as your article says, it is very easy for a hundred disparate groups to equally contribute to an insurgency, but very hard for them to unify into a governing bloc.
