Elective surgery just means it's not an emergency and can instead be scheduled for a future date. Medically unnecessary surgeries such as face lifts fall under this category, but even surgeries for cancer would be considered elective too because those are usually scheduled for a future date as well.

Around 80% of preauthorization denials are because the doctor or someone at the doctor office is retarded, like you and they did not send in appropriate documentation and almost all are resolved quickly once the office pulls it’s head out of it’s ass and submits the needed documentation.
Elective surgery is anything that isn’t an emergency/immediately lifesaving and isn’t cosmetic that doesn’t involve reconstruction from illness/injury. You just keep proving your retardation.
As to being a frequent user of services, I am not, nor have I been. However, I did save up to pay off my patient responsibility for having my surgery and post-surgery I no longer require meds for blood pressure, cholesterol, diabetes or pain for my knee and hips that were damaged from my mis-spent youth doing things like motocross, martial arts, farm work and tree cutting that hastened their decline.
I do have to ask, are you one of those that demeans people for going into rehab for alcohol or drugs since there are other ways to get off booze and drugs? Do you attack people that take methadone? By the way, there are many studies out there suggesting substantial cost savings for providing bariatric surgery rather than depending on other, much less effective ways to moderate weight due to the reduced cost of other care like diabetes and knee surgery.
