Maybe because medical documents (physical, not mental) are supposed to designate biological information, not an individuals desires or expressions.
Also, this is on the "birth certificate". Hence the "gender" is documented on the... "birth certificate".
I personally don't care what people want on it, but don't arbitrarily change something on a document that is designed to record biological information at conception. Expand the SS card information, or make a new comprehensive ID card that covers what things like that are for, for public presentation. Seems smarter than changing what is supposed to be a medical document recording information.
