It is a magical item which could be imbedded with a spell.
It provides a bonus of 11 to Pickpocketing Bonus.
This enhancement may not be used by adventurers who have not trained 44 times.
The earcuff looks to have a lot of charges remaining.
You can PULL the mithril alloy earcuff to remove it from the inventory.
