I guess this would be the right spot to ask this.
If I am picking in east tower.
I spot a scarab and dislodge it with one already on the ground.
When I disarm scarab will it disarm the one I dislodged? I am using an automated picker script so I am wondering if I need to change the scarab part to look for other scarabs on the ground first.

I would imagine just doing "disarm scarab" would disarm whichever scarab was dropped the most recent. So assuming one was on the ground then you drop one "disarm scarab" should work. However if you drop a scarab then someone else happens to drop a scarab before you can do "disarm scarab" you will disarm that person's scarab and there will be a live scarab on the ground.

If I recall, it has been a while since it happened, only the picker that dropped the bug can disarm it for a certain amount of time after it is dropped. Someone else can try to grab it and will get stung but not disarm. That may have changed it has been a while since I encountered it, but Tgo01 is right, the way items drop the most recent would be the one you interact with.

First, the east tower is a crappy place to pick. Bad lighting, and every idiot in the world will set off traps in that bad lighting and kill you.
Every new scarab that drops is the first scarab in the room order. You can't attempt to disarm another rogue's scarab unless you are in the same group.

Here's what I do... Keep in mind this is in Shattered.
I spot a scarab trap. I check the room for scarabs before dropping the one in the box. If there's one there, I'll wait for it to be gone, or about 30 seconds to a minute timeout. If it's still there after the timeout, I'll disarm it myself and grab it before proceeding. Might not be great etiquette in Prime. If I had to do it again, I'd probably go with room loot comparisons from before and after disarming the trap to filter out the scarab that's mine.
All that aside, yes, your scarab SHOULD be the first in the room, assuming nobody dropped one while you were in roundtime.

I've routinely dropped scarabs in quantity (4-5 often in a picking session) and have never had a "disarm the wrong scarab" sort of mishap. The only time I could see there being a problem is if you take one right as someone drops one or the like. As was stated, DISARM SCARAB will always disarm the most recent.
Though, the "first/second" adjectives and nouns used in this way don't ALWAYS work. They work almost all of the time, but there are some times where they don't. I know if you have one of the old heavy backpacks from the Icemule general store and you mix them with the new sturdy backpacks, you can have a bit of an issue in this regard. There are some situations where you MUST use adjectives. Scarabs is not one of those cases.
