Anyone have an explanation for this? Happened maybe 20 min ago.

I think it has something to do with CoL. I'm no expert but one of my pals did this once and that was the excuse he gave.

Maybe died very low on spirit and when his signs went off it spirit death'd him?

Or he died with a scarab in him and departed with it still inside.

Departed with scarab sounds like a good bet.

I died with a glaes heart in my hand and departed. Needless to say, I died almost instantly thereafter.

Yeah, people do that with weaponfire and scarabs do take a tiny bit before dropping out of the corpse too.
