is there a limit on the number of tattoos you can have? I have a bunch, but none currently on my thigh.

The number of tattoos you have shouldn't make a difference. If you really are completely naked when you ask I suspect it's a bug and you'll need to report it or assist.

Not always, until you do something to the weapon in the sheath.

Tattoos used to count as pinworn items, and there was I believe a 30 item limit. I don't know if this has changed but if you had any pinworn items removing them should have freed up a slot.
