When LOOKed at while held
The troll heart necklace is made of transparent glaes which allows you to view what looks to be the beating heart of a troll king inside the glaes. Near the bottom back of the necklace two small holes which have crusted over with a substance you have never seen before, sealing off the heart held inside.
You hang a troll heart necklace around your neck.
You are already wearing a troll heart necklace. (never BUGged this, since no one else sees it..)
(1-120 seconds later)
With no warning at all veins begin to grow through the back of the necklace, flailing wildly as they grow. There is a sudden burst of pain as you realize the troll heart necklace veins have punctured your flesh and have attached directly to your heart! You scream in pain!(knockdown, ~15 seconds RT, no stun)
when TAPped
As you tap the troll heart necklace you feel a slight irregularity in your own heart which skips a beat.
When health != max
The troll heart inside the necklace beats madly for a second as it pumps lifegiving blood back into your bloodstream, revitalizing you with its regenerative blood.
When health is full
There is a faint beating from the troll heart inside the necklace you wear.
LOOKed at or SHOWn while worn
The troll heart necklace is made of transparent glaes which allows you to view what looks to be the beating heart of a troll king inside the glaes. Near the bottom back veins exit the necklace which wind around the chain and vanish into your chest.
When attempting to remove without both hands empty
You try to remove the troll heart necklace but the veins which sink through the back of the necklace will not allow it to be removed so easily.
With both hands empty
Using all your strength, you rip the troll heart necklace away from your chest using both hands to do so. The veins that have sunken into the chest stretch to their maximum length before they snap in two leaking blood to the ground. You scream in searing pain as blood pours freely from the wound. Quickly the holes in your chest seal over.
The troll heart beats rapidly inside the necklace repairing the damage done to it from the action.
You take 12 damage. (No stun/RT)

As Brute taps his troll heart necklace, he seems to blanch in a most peculiar sort of way.

I paid a considerable amount when I bought this from Durgrimst a year or so ago. I don't think he owned it long or tested it at all. I based what I was willing to pay mostly on the fact that it's just very cool looking, and also on mechanical information provided by Khariz (the previous owner) here: http://www.krakiipedia.org/wiki/Troll_heart_jewelry_%28saved_post%29
At the risk of losing a good amount on this sale, here is the information I've gathered in testing.
The beating occurs at a set interval, whether your HP is full or not. The frequency with which it beats appears to be based on Constitution and/or PF ranks. With a CON bonus of 35 and ~290 PF ranks, it appears to beat every 120 seconds about 95% of the time, with the occasional missed beat, resulting in a 240 second gap between beats.
When testing with lower CON/PF characters, it did not beat as frequently. With a 11 CON bonus/30 PF ranks it was more often 4 minutes between beats. The longest gap I've seen between beats is 360 seconds. I never did any testing with a negative CON bonus.
Contrary to what what Khariz reported, I've never seen it pump more than 20 HP in one cycle. It seems to always be a random value ranging from 13 to 20. The amount of blood returned doesn't appear to be dependent on CON bonus or PF ranks. This is not to say Khariz' post is intentionally misleading, but I don't want anyone to buy this expecting 70 HP pumps. There are some factors that I have not taken into account when testing that may give over 20 HP. For example, I did most of my testing while only missing 30-40% of my max health, and my max health is only 159. Perhaps a Giantman with only 10 remaining HP could see a huge pump. There's also a chance that it's based on an open roll, and I've just never observed a 100+ roll activation.
More thorough testing could be done, especially in an attempt to determine if PF/CON are both relevant. I'm assuming that one of them is not.

Mechanics not considered, it's still way cooler than a forehead gem!
