I still can't decide. What do you think and why? I think I need to see it again, but it's not going to happen in the movie theater, so we have a while to debate about it.

I think it was a plot.
I don't really get how he was a dangerous, murdering psycho when his reaction to what his wife did, IMO, was not exactly crazy. I think I'd want to kill whoever murdered my children in cold blood, too, regardless of if they were my wife or not. It just seemed like major overkill.
Even for a Scorcese film, that movie was fucked up.

Interesting, I thought if it were a plot, then the whole wife killing the kids thing was planted by the Drs after giving him the drugs, and in reality she was killed by the fire, and they never had any kids.

Okay, he was crazy. That was pretty obvious to me. If you kill someone, and it's not out of self defense there's probably going to be a consequence. The fact that he was insane had him sent to the institution. The ending scene in the movie, once he realized that he was in fact crazy, and had to live with the knowledge that not only did he lose everything once important to him, but that he would also live out the rest of his days as a patient in a mental institution. Those two factors alone yielded the result that, fuck it, if I liked it better when I was crazy, and since I can't go back they might as well take part of my brain out. That way he gets his not so sweet release.
