I used to own that 5.5x poison flaring maul that Khariz selling. It got janitored and the GMs were able to get it back for me.

I should probably mention that it is paladin bonded so, if someone is just 'holding onto it' it won't work for them anyways.

Not that anyone would do that..

There is at least one Game Host that routinely exceeds his authority and takes it upon himself to make unilateral rulings on such things.

I would assist, get a referral to talk directly to a real live GM (not a game host) and just ask "Has this weapon been janitored, or is it still floating around the game?" Don't even ask for it back at this point. Just get the basic information so you can form a strategy to get it back.

If a player has it, you can try to get it back through in character channels, rewards, threats, etc.

If the janitor does have it, ask if the town guru would be willing to make that weapon part of a mini scenario to get it back. Involve other players, try to have fun with it, show that you're an interesting roleplayer and a good sport. GMs like helping fun people, and they like helping groups of fun people even more. I got a friends weapon back from the Great Gnoll (janitor) this way once.

I've also gone over the head of snarky Game Host when a confluence of separate systems bordered on the buggy to make me lose my own weapon. The GM answered about 5 hours later (probably to let me sweat a bit,) smiled and cheerfully handed me back a sword.

You are supposedly entitled to one lifetime no questions replacement- Andraste replaced something of mine once. In retrospect I wish I hadn't wasted it on something worth just a few million silvers, but it was a lot to me at the time. You might be able to do this if you really feel it's worth it.
