I didn't look at the video but no you don't have to consciously infect a zit for it to get infected. I accidentally scratched one open one time on my leg from an in-grown hair due to shaving, and I guess it got infected maybe from dirt under my nails or something. And I wash my hands a lot, so it doesn't always take much. I'm really careful about scratching now, lol.

Depends really. If you have a history of acne, then cysts like that aren't too uncommon (just sort of rare where she got hers). You can get really severe acne from folliculitis.
