I recently added the lovely map created by Lasentra for Halcyon Hills to Prime, but almost all of the memorials are unnamed. With help, I was able to find Fawn's memorial, and will add it to the map eventually. But before I do, I am seeking help from the community, to figure out the other memorials, so that those seeking to pay their in-game respects to their lost friends may do so.
I will list what Lasentra marked as possible memorials in Halcyon Hills, and if you recognize who it was created for, let me know and I will add them to the map.
1.) [Halcyon Hills, Lakefront - 14821]
Several hand-carved golden oak benches lie just before the shoreline, their wood badly weathered from time and long use. The two benches are positioned perfectly to provide a view of the no doubt breathtaking sunsets and sunrises visible over the tranquil lake.
2.) [Halcyon Hills, Lakefront - 14839]
Small waves roll slowly toward you, gently striking the grass at the lake's edge. Other than the gentle lapping of the waves and the distant birdsong from the north, no sound penetrates the tranquility of this lakefront. A single candle lies on the ground before you, its wick long burned to dust and its golden beeswax polished smooth by the water. Dozens more float in the distance, their tiny flames all dancing in the same rhythm.
3.) [Halcyon Hills, Forest Glade - 14835] - possibly Kali, can anyone confirm?
The northwesternmost corner of the glade is surrounded on three sides by oak saplings and thick brush. Further to the north, the forest becomes thicker. Older, even ancient, trees are visible through the sparse foliage. A carved granite gravestone rests just before the tree line, a line of short rose bushes planted on each side.

4.) [A Peaceful Garden - 14846]
The almost overwhelming scent of wildflowers mingles on the breeze with a hint of pine. A comfortable looking chair rests in the spot where the fence forms a corner to the northeast, and several pale green ferns are interspersed among the flowers. The loud babbling of the carved marble fountain drowns out almost any other noise.
5.) [A Peaceful Garden - 14847]
An ancient apple tree grows from the soft ground here, its gnarled boughs covered with dozens of tiny white flowers. The sound of singing birds fills the air along with the fountain's murmur, and the scent of flowers is almost overwhelming. You also see an ivy-covered silver fence.
6.) [A Peaceful Garden - 14848]
The grass in this garden is tall and full, occasionally trodden at the edges of the path. Wildflowers grow in several places amidst the grass and you even see a single cherry tree growing next to the fence. Sunlight sparkles brilliantly upon the water spraying from the intricately carved marble fountain.
7.) [Halcyon Hills, Aspen Thicket - 14864]
The trail widens into a circle beneath a thick cover of aspen boughs. There stands an immense arch, its pillars densely carved with sigils. The surface is pitted with time. Its incongruous location, beneath the graceful aspen canopy, implies the unpredictability of life's paths and the finality of the last gateway.
>look at arch
The stone arch is monumental in both its height and proportions. Richly carved sigils crowd its sides, comprised of an intricate, elven style and accented in places with the shapes of fanciful animals and elongated, stylized forms of elves and humans. On the ground beneath its lofty crown grows a rich carpet of low grass and moss.
8.) [Halcyon Hills, Serene Grotto - 14868] - Possibly Lady Wanton Destruction
The grotto is quiet, protected by the fieldstone walls that surround it and create an overhang above, stretching around to protect the eastern entrance with their lichen-covered arms. Water flows down the back of the grotto, tripping over the stones and falling into a dark pool beneath. The pool's surface ripples softly, hiding the secrets of its depths with a constant undulation. A statue stands at the side of the pool, its visage stoic and its secrets lost in time.
>look at pool
The pool is wide yet its perimeters do not crowd the interior of the grotto, its interior far surpassing the impression of its size from the outside. Ripples from the falling water traversing the back wall rock a number of floating candles, which dot the pool's surface like a tiny armada crossing a dark sea.
>look at statue
The figure is tall and its graceful head slightly bowed. Resting on the pommel of a magnificent sword, its hands are slightly crossed, with arms held loosely at its side. While the statue is obviously female, it projects all the finest qualities of both genders: strength, beauty, benevolence, and wisdom.
9.) [Halcyon Hills, Meadow's Edge - 14853] - Possibly GM Fawn
An imposing double row of poplars line a subtle lane that materializes out of grassy meadows like the beginning of a sweeping line of charcoal in an artist's sketch. Just at the lane's birth stands a statue. Depicting a doe, the sculpture lifts its head as though just surprised at something carried on the breeze, the animal's eyes large and imminently expressive. You also see some rough fieldstone steps winding up the bluff.
>look at statue
Carved from stone, the statue of a doe stands serenely within the meadow grass and wildflowers. The head is lifted, surveying the far horizon, and its ears are pricked. While the doe's posture suggests a brief moment of contemplation, the sculpture's classic style and weathered stone convey an ageless permanence.
10.) [Halcyon Hills, Meadow Trail - 14855]
A trail winds through mounds of wild flowers, which compete with the lush grasses inhabiting the meadow. To the west, the pastel green tones of poplar trees offer a lush contrast to the darker greens of a forested hillside behind them, and, to the south, the crystalline blue hues of a lake sweep the palette into refreshing, cooler tones. A single stone stele stands tall among the flowers, its sides rich with carved symbols.
>look at stele
The stele is carved from a very dark-toned stone, and time has weathered and pitted its four sides. It stands about five spans high, although carved sigils on its sides that are half-buried in the ground suggest that it might have settled and lost part of its original height. Near the top, it narrows to a point. On all four sides, carved symbols show a variety of expressively rendered animals sweeping up the stele in a grand spiral that suggests the world's creation and the onward rush of time.
11.) [Arboretum, Maple Bower - 14873]
Branches of bright green vine maples rise up from either side of the trail, arcing overhead and meeting in the middle to form a private bower. Dappled sunlight filters through the leaves, lighting the surroundings in a soft golden green as birdsong fills the air. A cedar bench provides a place for rest or private reflection.
>look at bench
The bench has been made from a cedar log that was split in two. The flat surface of one half provides the seat, and the flat surface of the other half provides the back, into which part of a poem has been carefully carved. The wood has been polished and then varnished to a smooth finish. Scattered around the bench are slim, curled wood shavings, as if someone has been whittling.
There appears to be something written on it.
>read bench
In the Common language, it reads:
"May your friends be true and loyal, may your enemies be few,
And whatever gods you follow send their sympathies to you.
May fantasy and wonder light the world before your eyes -
So long as we have magic words, the magic never dies."
12.) [Arboretum, Memory Boulder - 14875]
Statuesque deciduous trees of several varieties are scattered to either side of the wide dirt path, while lush green grass carpets the ground in between the leafy giants, creating an inviting park-like setting. The golden glow of sunlight filters through branches overhead, illuminating a small granite boulder with a green marble plaque set into it.
>read plaque
In the Elven language, it reads:
I hear your musical laugh as the brook rushes over stones...
I sense your strength in the solid feel of rock under my hand...
I hear your quiet whisper as the wind soughs through the leaves...
I see your graceful movements as the boughs of a willow sway...
I feel your soft touch as the warm breeze caresses my face...
I see your sparkling eyes as the sun glints off the blue waters...
...and I remember.
13.) [Garden, Fountain - 14881]
A large, many-tiered fountain immediately attracts attention, obviously intended as a centerpiece for the garden as it rises high into the air. The neatly tended beds surrounding the courtyard are full of pansies and other small, brightly hued flowers. Fancifully carved marble benches are set around the perimeter, providing a spot for visitors to rest and enjoy the display.
>look at fountain
The fountain is a tall spire with many arched arms gracefully extending out from its center. Each arm cups a basin into which water pours from above before cascading into another basin below. The beautiful piece is elegant in its simplicity of design.
14.) [Peaceful Meadow, Oak Tree - 14941] - Silverwolf, Reddwolf and Glima
Standing alone in the center of this peaceful meadow is a gigantic oak tree. Wildflowers have grown haphazardly throughout the grass creating an array of colors that spreads across the entire meadow. Off in the distance a small group of tawny deer can be seen grazing, with an enormous buck keeping watch. You also see a smooth gray granite bench with some stuff on it.
>look at bench
Smooth gray granite gives bulk to the well-worn bench. Gracing the seat at one end, a lean wolf lays quietly, the details of face and fur lined in hammered silver, and a great wolf has been crafted of polished red coral on the opposite side. Between these two kneels a shadowed figure drawn in ebon and onyx. Glinting darkly at his throat is a ruby fire elemental talisman. The word "Beloved" is hidden among the trees that form a subtle background for the two regal beasts and their shadowed friend.
15.) [Oak Tree, Top of the Tree - 24687]
Atop this gigantic tree, an oak platform has been constructed. A colorful array of flower petals is scattered over the floor, occasionally whirling about the air whenever a gust of wind rushes through the tree top. Spread about the platform are several benches, providing ideal locations for contemplation and reflection. Near each bench is a handcrafted wicker basket filled to the brim with petals. You also see a small sign.

In the Common language, it reads:
Feel free to toss your petals from this platform.

In the wicker basket you see a handful of violet rose petals, a handful of black rose petals, a handful of blue flower petals, a handful of soft lilac petals, a handful of pale pink rose petals, a handful of yellow rose petals, a handful of white rose petals and a handful of red rose petals.
16.) [Remembrance Ridge, Promontory - 14890]
Jutting out from the promontory of the end of the ridgeline, this overlook of barren rock provides a spectacular viewpoint. Placed here for those wanting respite or a moment to enjoy the scenery, a crude wooden bench faces the northwest stretch of the valley. At the tip of the bluff, a break in the treeline reveals a switchback trail leading down the hillside. You also see an arching stone bridge.
17.) [Memorial Cavern - 14894]
The narrow entrance opens up into a small, dry cavern. Many niches are carved into the solid rock walls, and stone biers ring the roughly circular floor. Candles set into elaborate mithril sconces illuminate the area, but the ceiling is high enough to be lost in darkness. In the center of the cavern is a large stone altar that is chiseled into the shape of an anvil.
>look at niches
Many of the niches contain large stone urns in which bones of the dead have been stored. The urns vary in artistry, some rather plain and some elaborately carved. Quite a few of them have small empty spots that were once probably inset with gems.
>look at biers
Some of the biers contain sarcophagi of marble which are intricately carved with various designs. Most of the lids have been removed or slid partly aside. Other biers contain the desiccated skeletons of short, heavily boned people. Many of the skeletons have been disturbed, and any treasures that were left with the dead have been removed.
>look at altar
You see nothing unusual.
>look on altar
On the stone altar you see a small rose, a small pumpkin, a lump of wax, some cave moss, a wreath and a thin sharp steel dart.
18.) [Dome of Memories - 14897]
Frost-covered trees greet visitors to the garden, each of their leaves lightly dusted with a glittering layer of snow. Fragile icicles dangle from the branches, many long enough to reach the ground. A lone wooden bench faces the tallest of the trees and looks to be an ideal spot for contemplation.
19.) [Dome of Memories - 14899]
Decorative crescent openings in the ceiling grant a view of the sky above and create a pretty pattern of shadows upon the floor as the sunlight overhead filters through. A small white bird trilling a pleasant song is perched nearby, quite at ease in one of the slivered vents. You also see a carved wooden bench.
>look at bench
Crafted from fel, the bench seats two perfectly.
20.) [Dome of Memories - 14905]
Tiny yellow and white flowers blanket both sides of the small stream cascading across this ice-covered field. A rainbow of butterflies hover about the flowers, their movements creating an everchanging portrait of color against the snowy white backdrop. A pair of curved wooden benches face each other, allowing for soft conversation or peaceful meditation.
21.) [Contemplation Canyon, Arbor - 14912]
Grey-green serpentine rock streaked with white quartz and flecked with silvery mica is carved to form a low three-sided wall resembling a leafy bower. Ivy-twined poles at the four corner points shoot up to suspend a canopy of sheer gossamer. Two baskets of draping fragrant jasmine add just the right touch to the peaceful setting. From this vantage point, several other arbors are visible across the wide meadow, as is the narrow ridge of trees that hides the ascent out of the canyon. You also see a grey-green stone bench.
22.) [Contemplation Canyon, Trail - 14929]
A small riverstone cairn sits alongside the trail here, but, rather than serve as a burial mound, the rocks provide a bier for the recently succumbed and mourned. Trees and brush have been cleared away, creating a small amphitheater for the grieving in attendance.
23.) [Contemplation Canyon, Arbor - 14928]
The trail breaks at this junction, circling a white marble arbor veined with pale pink granite and vibrant carnelian. Cream-hued passionflowers with deep purple centers climb the four corner pillars and creep along the open-weave ora mesh above. The full blossoms release their exotically sweet and comforting scent. Following the path of the sun, they welcome the golden rays that light up their pollen-coasted pistils like tiny golden charms. You also see a white marble bench.
24.) [Contemplation Canyon, Trail - 14925]
Circling perfectly here is a small stand of aspens, their green and golden spade-like leaves fluttering in the breeze. At the center, a large river stone, its top flat like an altar and holding many offerings from those who have visited. The trail continues behind the grove, running alongside the creek that rings the valley floor.

25.) [Leaping Falls, Basin - 14923]
Tall and graceful, this triple-step waterfall cascades from tier to tier, moving down the cliff wall in gentle leaps. Rock shelves extend out, catching the thin streaming spray in a shallow pool that spills over the downsloping side. Twice more the falls are snagged, until it washes down the remaining canyon wall, to constantly renew the clear basin. You also see a small pile of pale iridescent pebbles and a small wooden bridge leading back to the main trail.
>look at pool
The quiet waters of the pool are mirror-like still, were it not for the occasional concentric ripple. Images and memories from long ago, take on a misty form across the surface, then fade quickly, as if they were never there. A soft glow emanates from the crystal blue waters and illuminates the depths below, revealing a menagerie of various pebbles, from those who have come and gone.
>look at pebbles
You see nothing unusual.
26.) [Contemplation Canyon, Arbor - 14920]
Standing out from the main trail, columns of grey-blue marble streaked with pyrite and serpentine support a pale haon cross-beam roof. Spiraled cypress trees stand tall, extending out like wings from two sides of the arbor. Nearby, the soft sound of Leaping Falls serves as a backdrop to the view of the open meadow extending out in front. Soaring hawks dive and turn, riding the thermals rising up at the canyon walls. You also see a grey-blue marble bench.
27.) [Contemplation Canyon, Arbor - 14914]
Pink marble columns streaked with white quartz, black obsidian, and flecks of silvery mica hide beneath a lattice roof thickly covered with honeysuckle vines. Sheltered by the trees that hug the creek at the base of the cliffs, the small arbor is protected from the bright sun much of the day. The susurrus of rushing water nearby marks a small trailhead to the south. You also see a pink marble bench.
